It uses AdLib True Type font in the artwork, which is a copy of Addled. This is another entry in a DTP Template Contest for November 1994.
To use: Load the file as a template. Go to FILE, NEW FROM TEMPLATE, and the form will be ready to go. Select a group of information as defined by the blue guidelines and go to TEXT, EDIT TEXT, and enter your own information.
